Driving Directions
Our Pueblo office is located on the south side of Pueblo. We are in the Plaza Del Sol center on Moore Ave., one block West of Prairie and Northern near South Side King Soopers. Please call us for directions.
In La Junta, Walsenburg, and Canon City, we see our patients at offices located within the hospitals in each community. We schedule all patients through our Pueblo office. Specific directions to help you find our clinics at Arkansas Valley Regional Medical Center in La Junta, Spanish Peaks Regional Health Center in Walsenburg, and UCHealth Parkview Specialty Clinic in Canon City can be obtained by calling our office at (719) 564-2391.
Insurance and Billing
We are very conscious of patient costs. Because many of our patients are experiencing greater out-of-pocket payments, this office is dedicated to providing quality care in a cost effective way.
We are participating providers with Medicare, Medicare Advantage Plans, and most major health plans. The larger plans are listed below. We collect co-pays and deductibles. We do ask for these at the time of service. We remind our patients that they have the ultimate responsibility for services received. This includes responsibility for obtaining a referral or prior authorization from your primary care provider if required by insurance.
